Position Summary

The primary purpose of your job position assists residents in the community with all activities of daily care and services in accordance with the resident’s plan.

Essential Functions

• Provide assistance and general resident care, including, but not limited to: social interaction, meal assistance, bathing, toileting, and housekeeping
• Follow care plan, including monitoring resident inputs, outputs, vitals, behavior, safety concerns and any other changes
• Upon request, transport residents to and from required appointments
• As needed, be an active participant in life enrichment activities

Documentation

• Contribute to employee communication log to ensure smooth transition of necessary information from one shift to the other
• Document all inputs, outputs, vitals, behavior, and/or safety as needed in the client file

Preferred Qualifications

• Previous experience working in a memory care or assisted living community

Physical Working Requirements

• Ability to travel using personal vehicle, including at night
• Able to do occasional lifting of up to 50 lbs.
• Able to work flexible schedule, including evenings and/or weekends
• Team Member is subject to outside environment conditions occasionally, inside conditions frequently, with protection from weather conditions but not necessarily from temperature change
• Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious diseases, air contaminants, and hazardous chemicals
• Must be able to read, write, speak and understand English
