Responsibilities:
- Assist individuals with daily activities such as meal preparation, personal hygiene, and medication management
- Provide emotional support and companionship
- Implement individualized care plans and monitor progress
- Promote independence and encourage participation in social and recreational activities
- Maintain accurate documentation of services provided
- Collaborate with a multidisciplinary team including social workers, nurses, and behavior management specialists

Skills:
- Experience in meal preparation and ensuring dietary needs are met
- Knowledge of social work principles and practices
- Ability to provide patient care with empathy and respect
- Familiarity with dementia care techniques
- Understanding of HIPAA regulations and maintaining confidentiality
- Direct support experience in caregiving or related field is preferred
- Strong communication and interpersonal skills
- Ability to effectively manage challenging behaviors
