Primary Purpose of Position: 

The role of the MDS Coordinator is to plan and organize the MDS process along with the MDS Director, and interdisciplinary team members. This position assures compliance with all State and Federal MDS date sensitive and transmission requirements. The MDS Coordinator is responsible for collaborating with Clinical and Administrative, Therapy and billing staff regarding issues relevant to the Minimum Data Safety Set (MDS 3.0) to promote efficient and accurate coding of MDS for Medicare/Medicaid and other payer types to maximize reimbursement as State and Federal Laws permit.

Job Duties:

  • Works along with the MDS Director for assignment, scheduling, proper assessment and coding of all admissions, discharges and quarterly or more often.
  • Completes the MDS 3.0 assessment, MDS paperwork, electronic files and transmission protocols within the required time frames and initial basic care planning (CAA’s).
  • Completes assessments that are accurate with proper coding.
  • Consults with IDT members for pertinent clinical information as needed.
  • Utilize direct interview/observation skills, chart audits, electronic record keeping data resident/family interviews and communication with direct care staff when completing assessments.
  • Works with interdisciplinary team to determine resident significant change in condition and OMRA assessments.
  • Completes assigned sections of MDS per facility policy and ensures completion of input in assigned sections by other disciplines.
  • Creates, follows and updates facility MDS schedule and tracking/discharge forms per policy and procedure.
  • Follow facility policy and procedures on CAA’s and care planning.
  • Ensure that care plans accurately reflect the cares and clinical monitoring provided.
  • Audits as assigned by MDS Director and/or contracted consultants.
  • Distributes copies of audits for correction to appropriate departments and copies Administrator.
  • Monitors regulatory compliance for completion of MDS 3.0.
  • Notifies MDS Director, department supervisor(s) of significant or persistent problem areas.
  • Coordinates the completion of Admission, Discharge Tracking Forms and Return tracking forms as required.
  • Tracks completion of MDS’s and transmits in compliance with HCFA timelines as delegated by MDS Director or per facility policy.
  • Reviews Validation reports and makes appropriate corrections, and reports issues to MDS Director.
  • Informs MDS Director and IDT members of validation errors as appropriate.
  • Follows the HCFA Correction Policy for corrections.
  1. Uses Quality Indicator data and other reports to improve MDS accuracy and compliance
  • Prints Quality Indicator Reports as delegated per policy.
  • Reviews and distributes as directed.
  • Assists IDT in determining MDS accuracy issues related to Quality Indicators.
  • With appropriate IDT members, develops and implements appropriate action plans to correct MDS accuracy issues.
  • Confers with MDS Director, contract/consultant specialist to gain direction or clarification as HCFA determines.
  • Works/ confers with MDS Director, Billing Office to ensure accuracy in billing.
  1. Personnel functions
  • Supports Administration in assisting other campus in related duties and relief for vacation/illness or as needed as assigned by MDS Director.
  • Attends ongoing educational offerings of topics pertaining to assigned duties to remain current in areas of responsibilities.
  • Provides educational training relating to Medicare, MDS, and care planning process to staff as requested.
  • Attends educational training relating to Medicare, MDS and therapy related needs.

Qualifications


Experience: AANAC Certification for Resident Assessment Coordinator- certified (RAC-CT) is desired.
Education: High school diploma or equivalent. 
Registered Nurse with previous long-term care experience with working knowledge of the MDS process.
Other: Must maintain complete confidentiality and discretion. 
Read, write and speak the English language.
