Outreach is hiring a Full-time Clinical Case Manager at Jubilee Village—Carol Stream to strategically support the residents as they journey towards their goals and self-sufficiency by providing case management as part of a collaborative team of professional staff and volunteers. Responsibilities: The Clinical Case Manager will manage a caseload of 8-10 therapeutic casework clients. The Clinical Case Manager will work with clients to set goals related to employment, making and adhering to a budget, completing schooling through high school or beyond, seeking counseling for mental health needs, life skills, leadership development, and providing necessary transportation. The Clinical Case Manager will connect residents to resources and advocate for residents with other service providers. The Clinical Case Manager will report to the Jubilee Village Program Director and participate as a member of the broader transitional housing team. Schedule: Monday – Friday, 10:00 am to 6:30 pm. Two to three evenings a month for life skills training at team meetings; other occasional evenings or weekends to meet client needs. Qualifications: MA/ MSW with at least 1 year of case management experience, or BA/BSW with 1-5 years of experience in case management. Demonstrated success as a case manager. Experience in working with marginalized populations and ability to develop effective relationships with participants and outside service providers. Passion for guiding young women to envision a successful future. Resilient, active listener, self-starter, and initiator. Ability to work independently, as well as collaboratively with a team.
