Outreach Community Health Centers is Hiring a Licensed Professional Counselor – Clinical Coordinator Near Milwaukee, WI

JOB PURPOSE AND REPORTING STRUCTURE: Under the direction of the TCM/CCM Manager, the TCM/CCM Clinical Coordinator oversees the quality of treatment services delivered to chronically, mentally ill clients (as diagnosed using DSM IV criteria) within a program of assertive community treatment. The TCM/CCM Clinical Coordinator performs direct clinical supervision of the treatment team including the screening and admitting of new clients to TCM/CCM, in accordance with agency policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

  • Ability to assess a variety of AODA problems such as abuse and / or dependency issues and to effectively attend to clients’ needs using a variety of treatment modalities such as Cognitive Behavioral Therapy, Harm Reduction, and Motivational Interviewing.
  • Knowledge regarding to use of psychopharmacology for the treatment of a variety of client conditions.
  • Interviews clients to complete Mental Health/AODA functional screen and develop an individual recovery plan, to include case planning, obtaining referrals for services, on-going monitoring, modeling and service coordination.
  • Assist clients developing client centered goals and services such as medical and mental health assistance, obtaining legal assistance, benefits, medication management, finding employment, training and education, financial management and budgeting, ADL assistance, development of social support systems, AODA services and support, etc.
  • Manage minimal caseload, provide supportive contacts and assist clients in navigating through the community.
  • Intake clients from beginning referrals and meet clients where they are in the community, to include hospitals within established guidelines.
  • Provide services such as care coordination, medication monitoring, physical health monitoring, individual skill development and enhancement, psychoeducation, wellness management
  • Assist clients with developing life skills helpful for independent living.
  • Complete discharge paperwork if client meets predetermined discharge criteria.
  • Transfer clients when services are no longer required or if other level of services are needed.
  • Attend TCM/CCM Operation meetings for transfers or discharge of clients.
  • May coordinate/provide transportation for clients to and from appointments, to include medical and mental health, shopping, housing, etc.
  • Attend and actively participate in departmental meetings and treatment team meetings with other service agencies/supports.
  • Complete monthly billing for T-19 reimbursement of services provided to each client.
  • Ability to assess and intervene in crisis situations that may result in client harm to self or harm to others in the community.
  • Provide clinical supervision to the TCM/CCM treatment team, (one-to-one, team and side-by-side), ensuring the best possible services to clients.
  • Ability to diagnose new admissions to the TCM/CCM program and to differentiate DSM IV diagnoses. 
  • Screen and admit clients to the TCM/CCM program, following procedures.
  • Oversee the quality and frequency of treatment services provided to TCM/CCM clients, to ensure meeting the agency's standards.
  • Complete the assigned sections of the In-Depth Assessment. Develop the initial plan for services at the time of admission.
  • Review all treatment plans and in-depth assessments for TCM/CCM clients, ensuring the meeting of agency standards.
  • Assign clients to case manager caseloads.
  • Provide case management for assigned caseload.
  • Represent the agency at outside meetings, with funding sources and with the state regulatory agencies.
  • Maintain technical competency and remain current in technology and changes in the industry.
  • Complete and maintain all required paperwork, records, documents, etc.
  • Follow and comply with all safety and work rules and regulations. Maintain departmental housekeeping standards.

EDUCATION and/or EXPERIENCE: Requires a master's degree in a job related major and three to five years of related experience including 3000 hours of clinical post master’s degree supervised hours; or a combination of education and experience.
