The work of a Direct Support Worker is directed by a plan of care developed by the Oregon Department of Human Services.

The direct support worker follows the individual service plan for each individual.

Client 1 :

• Female 11yrs old Gresham 97080
• Desired Schedule : Wed & Thur 3pm (3-4 hr shifts) Fri all day
• Likes : Being active, playing outside, very imaginative, and very adventurous
• Needs : Energetic and to be excited to take client out into the community and to events that would fit what the client would like and enjoy as well.

Must be able to do crafts etc to work on her manual dexterity and and fine motor skills.

Connect more with females than males.

Client 2 :

• Female 8yrs Gresham 97030
• Desired Schedule : Mon-Tue 3pm-7pm , Wed 3pm-4 : 30pm, Sun 1pm-7 : 15pm (can do mornings if afternoons dont work)
• Likes : She loves cars, koalas and noise.

Her family calls her the Queen of Chaos.

She loves to laugh and gives kisses and hold hands.

She likes action movies and Herbie the Lovebug.

Needs : Help her to continue to learn and use her AAC and continue to grow her verbal communication skills as well.

Increasing focus and independence.

Work on impulsivity.

Client 3 :

• Female 10yrs Gresham 97080
• Desired Schedule : Mon-Fri 2 : 30pm-5 : 00pm, Saturday between 10am-2pm (Flexible)
• Likes : Loves music, plays soccer and piano, loves playing with baby dolls, loves helping around the house and being with adults, loves playing tag and swinging on swings
• Needs : Accompany to activities, gymnastics or piano lessons.

Someone who is willing to help client get more active and take her on a hike, bike ride, swimming.

Client 4 :

• Male 1 yrs Troutdale 97060
• Desired Schedule : Day Shift : Mon- Fri 7a,-12pm (or later if the hours are wanted) Night shift : Sun- Thu 10pm-6am
• Likes : Sweetest little boy.

Has a trach NOT vent dependent and a G Tube, enjoys playtime and snuggles.

He loves dogs and adventures and getting out and about

Needs : Both Night shift and day shift, someone consistent, committed, someone willing to get Nurse Delegated once hired.

Client 5 :

• Female 11yrs Gresham 97080
• Desired Schedule : Mon, Tue, Thur, Fri, 3 : 00pm-6 : 00pm, Wed 7 : 30am-9 : 30am and 3 : 00pm-6 : 00pm, Sat 4 : 00pm-8 : 00pm (once a month)
• Likes : Play Ukulele, sing and have fun.
• Needs : Needs to work with her on her daily tasks, hygiene, taking her out into the community

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following.

Other duties may be assigned.

• Deliver training, support, and assistance to the clients in the areas of personal care, recreation and leisure, daily living skills, social skills, health needs, personal preferences, exercising of individual rights, community integration, and behavior management.
• Participate in the implementation and monitoring of the clients individualized support plan.

Your observations and opinions are crucial to tailoring an achievable plan.

• Ensure clients health and safety at all times including but not limited to prevention of abuse, perform CPR, perform Standard First Aid, and other needs that the client may stand in need of.
• Work directly with the client / family of the client to ensure the goals of the client are being met and being worked towards
• Cooking, cleaning, medication assistance, scheduling, attend medical appointments, shopping, and more...
• Skill development and support is defined in the individual’s Individual service plan
• Documentation of progress notes with each shift

Qualifications and Education Requirements :

• High school diploma or equivalent
• Personal Support : 1 year previous experience (required)
• Transporting individuals
• Be certified in CPR and First Aid prior to clocking a shift
• Overall competency in working under pressure
• Experience in providing general care and support
• Knowledge of home care services
• Proficient in written communication
• Proficient in oral communication
• Be at least 18 years of age
• Understand requirements of maintaining confidentiality and safeguarding individual information.
• Be literate and capable of understanding written and oral orders.
• Be able to communicate with individuals, health care providers, case managers, and appropriate others.
• Be able to respond to emergency situations at all times services are being delivered.
• Must have a valid Drivers License, Auto Insurance, and reliable transportation in compliance with the laws of the Department of Motor Vehicles.
