Responsibilities:
- Provide direct care and support to individuals in their homes or residential facilities
- Assist with personal hygiene tasks, including bathing, grooming, and dressing
- Administer medications according to prescribed schedules and document accordingly
- Monitor vital signs and report any changes or concerns to the appropriate healthcare professionals
- Assist with meal planning, preparation, and feeding as necessary
- Support individuals in participating in social activities and community integration
- Collaborate with other healthcare professionals, such as social workers and hospice care providers, to ensure comprehensive care for individuals
- Document all care provided accurately and maintain confidentiality of sensitive information
Qualifications:
- Previous experience in home care, childcare, or caregiving is preferred
- Knowledge of medication administration, first aid, and basic medical procedures is a plus
- Familiarity with activities of daily living (ADLs) and ability to assist individuals with these tasks
- Strong interpersonal skills and the ability to communicate effectively with individuals of diverse backgrounds

- Ability to work independently as well as part of a team
