Our Ladys Inn is Hiring a Family Specialist Near St. Louis, MO

Description

General Summary:

The Family Specialist has the responsibility for advocacy and case management for assigned clients in Our Lady’s Inn’s residential program. He or she will work with clients to provide continued case management, resource referral and general support. The Family Specialist assists the family to a path of health and independence. The Family Specialist must have a commitment to the agency’s pro-life mission.

Schedule: Should be able to work 1-2 evenings per week to support the clients' schedules.

Essential Responsibilities: (include but are not limited to):

  • Performs intake assessment and orients new clients to Our Lady’s Inn program.
  • Enters client’s family demographic information, intake and exit assessment scores and all required information in HMIS and Life Force databases.
  • Coordinates in-house class schedules with clients’ schedules and needs.
  • Establishes and maintains a therapeutic relationship with clients.
  • Guides each assigned client on developing a Person-Centered Plan and provides support to the client on completing goals.
  • Provides input to the program team regarding clients’ Person-Centered Plan, including progress and recommendations for needed resources or interventions.
  • Conducts case management assessment and document transition plan at exit.
  • Records case management activity on each client.
  • Monitors client attendance at work, school or programs.
  • Coordinates with Child Advocate on the support plan for clients’ children.
  • Works with support staff to oversee completion of clients’ performance of daily duties and instruct clients in appropriate methods to complete. 
  • Works cooperatively as a team member with all staff.
  • Attends weekly staff meetings and all-staff meetings.
  • Assists clients in securing transportation for appointments and accompanies them to necessary destinations.
  • Assists with front office duties such as answering phones and greeting clients, staff, and visitors as necessary.

Requirements

Education: Bachelor’s Degree in social work, counseling or related field required; Master’s Degree preferred.

Experience: Experience in social service field with direct client interaction. Case management or counseling experience. Experience with trauma informed care service delivery and homelessness preferred.

Skills and Abilities: Organized and detail oriented; ability to establish professional boundaries and maintain therapeutic relationship with clients. Must have the ability to work collaboratively within a team environment. Excellent written and verbal communication skills; and must have proficient computer/database skills.
