Our Lady of Wisdom Community Care Center is seeking an LPN or RN MDS Coordinator!

Job Responsibilities:

• Provides each resident with professional nursing care, assessments and evaluations.
• Responsible for the management of nursing activities performed by Neighborhood/Household (NH/HH) staff ensuring that the highest degree of quality care is maintained at all times
• Works with a team of cross-trained staff to coordinate the functions of the NH/HH
• Has 24 hour responsibility for clinical functions of the NH/HH

Qualification:

• A current, valid Louisiana registered nurse (LPN) license is required
• Minimum of one (1) year experience in long term care, prior MDS experience is preferred.
• Ability to effectively communicate, direct and delegate tasks.
