Our Hope Association is Hiring an Intake Coordinator Near Grand Rapids, MI

Overview:

Interested in working with the recovery community? We are seeking a highly organized and detail-oriented individual to join our team as an Intake Coordinator. As an Intake Coordinator, you will play a crucial role in ensuring the smooth and efficient transition for clients of our residential treatment facility. The Intake Coordinator is responsible for supporting the Intake Manager by performing a variety of duties and projects related to the client intake process.

Essential Responsibilities:

  • Evaluate client's eligibility for admission under the program criteria for intake.
  • Obtain authorizations for admissions from insurance/managed care company.
  • Make sure client is admitted to treatment with all required items: medications, identifications card, and insurance card.
  • Complete the entire intake process, including all required forms, belongings search, and client orientation to treatment.
  • Work with the Clinical Team to assign rooms and assist the client in transitioning into the program.
  • Serve as a client advocate.
  • Maintain case documentation (e.g., collect and enter data, progress notes and all other relevant information to assist clients and to comply with program requiremments).
  • Complete intake note for client upon admission into treatment.
  • Make continual contact with women on the waiting list, based upon their needs.
  • Maintain report and notes related to the status of the women on the waiting list.
  • Possess knowledge of substance use disorders, mental health conditions, physical health limitations, and behavioral health concerns as they relate to admission into the program.
  • Assist in the preparation of quality improvement activities and required reports.
  • Perform monthly billing integrity audits.
  • Assists with the monthly billing by entering data into the system as needed.
  • Assist in the processing of medication scripts and pick up. Assist in the monitoring of medications as needed and directed by management.
  • Respond to all "Get Help" inquiries and manage the call log.
  • Maintain compliance with state and federal employment regulations.
  • Continuously drive and support a healthy and safe culture.
  • Maintain competency in policy rules and regulations.
  • Provide educational, didactic group sessions for clients, when needed.
  • Utilize the Insync EMR (Electronic Medical Record) for client record keeping.
  • Practice and uphold the Our Hope Standards of Excellence and Ethical Codes of Conduct.

Education & Experience:

  • Preferred Bachelor's Degree in Social Work, Human Service, or Equivalent field with appropriate licensure
  • Preferred one (1) or two (2) years of experience in chemical dependency and mental health.
  • Preferred CADC or CAADC
  • Current certification in First Aid and CPR
  • Valid Driver's License

Other Knowledge, Skills, & Abilities:

  • Demonstrated skills in working with clients from diverse backgrounds.
  • Knowledge of gender specific treatment interventions
  • Demonstrated ability with word processing software and office equipment
  • Organizational and time management skills
  • Good oral and written communication skills

Job Type: Full-time

Pay: From $39,000.00 per year
