OVERVIEW

Otterbein at SpringMeade Health Center is looking for a Full time Nurse Navigator/Wound Nurse is to facilitate residents through the discharge process, coordinate discharge through transition from facility to home and continuing through 30 days post discharge. Provides physician ordered wound care/treatment to all residents. The successful candidate will coordinate and promote the facilities services to Home Health Agencies. Discharge Planners, Case Workers, Primary Care and Specialty Physicians.

Shift: Full-time M-F 8am - 5pm

Responsibilities

• Navigate residents through discharge process.
• Collaborate with Social Work, Nursing, community resources and patients for smooth transition back home/community for safe and efficient discharges.
• Develop and maintain close working relationship with Physicians, Home Health Agencies, Hospice Services and hospital Case Managers throughout the discharge process and beyond.
• Ensure all orders, equipment, special needs and patient education has been completed prior to discharge. Ensure home health agencies perform an onsite visit and goals are met/set.
• Follow patients closely 30 days after discharge to ensure success in the community.
• Coordinate Physician follow up visits with Primary Care Physician and Specialists.
• Ensure both Primary Care Physicians and Specialists have discharge information the day of discharge and coordination of care with family.
• Responsible for receiving referrals and inquiries from former patients through home health.
• Support the Sales and Business Development Director with sales calls associated with discharge planning.
• Submit timely and accurate expense reports within budget allotted.
• Maintains forms, reports, and statistical data including input into admission tracking application.
• Tracks discharge calls, visits and referral activity daily/weekly.
• Assists the admission department with arrangements and confirmation of all readmissions back to the facility.
• Understanding of insurance contract content for need to obtain prior approval from authorized payer.
• Completes records and documentation in accordance with Company policy and State an/or Federal guidelines.
• Assessing and monitoring wounds.
• Cleaning and bandaging wounds.
• Bandaging the wounds by creating a barrier against microorganisms and changing the dressings regularly.
• Providing wound VAC’s when necessary.
• Writing orders to accelerate wound healing and prevent skin disintegration.
• Working collaboratively with other members of the patient care team to determine if additional treatments or changes in care are required.
• Educating patients and caretakers on wound care, infection and injury prevention, and pressure ulcer (bed sore) care.
• Other duties as needed.

Qualifications

• Registered Nurse or BSN.
• Current licensure by the Ohio State Board of Nursing.
• Three (3) to five (5) years experience in healthcare preferred.
• Familiarity of area hospitals, physicians and senior community organizations is a plus.
• Knowledge of the discharge process for skilled nursing facilities is required.
• Knowledge of Medicare, Medicaid, Commercial Insurance, and private payor sources.
• Related healthcare experience and knowledge preferred.
• Self-directed and very organized with the ability to work with little supervision.
• Compliance with accepted professional standards and practices.
• Excellent communication (written and verbal0, negotiation and public relations skills.
• Demonstrate history of positive outcome results, preferably involving sophisticated and meaningful customer relationships that lead to outstanding results.
• Ability to develop outstanding relationships with Home Health, Hospice, Physician Practices, and other resources in the community.
• Wound VAC experience.
• Computer efficiency is required.
