Essential functions of this job include, but are not limited to:

• Maintains confidential, accurate, and up-to-date case file for clients.
• Administers problem identification sheet/self assessment for clients who were formerly incarcerated in an effective and accurate manner.
• Identifies clients needs including, but not limited to: housing, substance abuse treatment, mental health services, medical services, mentoring services, family assistance and services, counseling, employment training, adult basic education or other educational needs.
• Coordinates with services providers to provide linkages for services for identified client needs in an effective, efficient, and courteous manner. Accurately documents information for client case file.
• Makes referrals to community organizations for clients. Follows up with clients to ensure services were provided. Intervenes and acts as an advocate for clients when problems or issues arise as needed.
• Closes clients file in a timely manner after verifying client services were received.
• Reactivates client files if clients return to program for further assistance.
• Assists individuals who seek information for themselves and/or on behalf of someone else who has had involvement in the criminal justice system.
• Provides the “Returning Home Guide” from the Office of Reentry to clients or individuals seeking information on behalf of someone who has had involvement in the criminal justice system. Orients and assists individuals with how to use the guide to identify services for themselves or others. Makes phone calls when possible for assistance.
• Provides exceptional customer-service to clients and contribute to a team-focused, productive, positive, and therapeutic environment.

QUALIFICATIONS: High School diploma or GED and a minimum of two (2) years of experience with a social services organization or Associate’s degree in social services or a related field required. Must have exceptional interpersonal skills, excellent organization skills, and the ability to remain customer-service focused at all times. Must have the ability to work effectively with Agency employees, outside contacts, and a diverse client population.
