Orchard Place is Hiring an Integrated Health Program Supervisor Near Des Moines, IA

The Integrated Health Program (IHP) Supervisor functions as a key member of the overall IHP Care Team by assisting the PACE Vice President, Clinical Director, and IHP Director in providing direction, program supervision, training, coaching and support for other members of the care team. The IHP Supervisor works to assure the Integrated Health Program is carried out in accordance to funding requirements. The IHP Supervisor leads high performing teams through quality improvement and assurance; implementation of operational procedures; and coaching professional development of staff.

What you will do:
  • Adheres to the philosophy, values, commitments and practices of the Sanctuary model in guiding the day-to-day interactions with clients, families and staff.
  • Respects and values individual differences and actively supports a culture that embraces diversity, equity and inclusion.
  • Provides supervision and leadership to various staff across IHP programs that includes OP supervision core components. Facilitates the creation of high performing, effective care teams.
  • Coordinates orientation of new staff, initial and ongoing training, coaching, and support.
  • Coordinates program development related to meeting deliverables with the IHP Director.
  • Provides supervision of staff that ensures fidelity to each program model and contractors.
  • Provides data oversight to assure that care team members satisfactorily complete timely and accurate service documentation, monthly billables, and reports as required.
  • Fields and responds to communications from external providers related to enrolled members. Problem-solves member issues related to resources, flex fund requests, and collaboration.
  • Participates in marketing and outreach for IHP referrals and maintains relationships with referral sources.
  • Monitors Medicaid status of clients and ensures reconciliation of MCO monthly enrollment rosters.
  • Assists with obtaining information for annual reenrollments
  • Ensures compliance with MCO-specific implementation processes that includes achieving incentive measure goals.
  • Ensures staff understand how specific roles align with organization and department mission.
  • Participates in PACE management and SMT meetings.
  • Participates in regular supervision with the IHP Director.
  • Carries a manageable caseload of clients. Conducts individualized, comprehensive, whole person health assessments.
  • Provides planning and care functions such as: follow-up contacts with the family and community partners; coordinate and triage referrals; and work with referral sources and agencies.
  • Fosters a care team climate that respects the culture, diversity, values, and preferences of children/youth, families and communities served.
  • Understands and applies the principles and practices of trauma informed care approach that recognizes strengths and inspires hope.
  • Participates and completes training on health home model and practice, and other relevant training.
Sound like a good fit? A few requirements include:
  • Bachelor's degree in human services related field and experience providing mental health services for children, youth and families.
  • Two years of supervisory experience preferred. Program coordination and leadership experience also preferred.
  • Ability to to communicate effectively verbally and/or in writing and have knowledge of and the ability to implement the principles and approaches to care coordination.
Criminal, abuse registries, and motor vehicle records checks completed.
