Position available for:

  • Oakland County

The Hospice RN provides and directs the provision of home hospice nursing care, based on agency policies and procedures, through the competent application of the nursing process.

  • Minimum of Associate's degree in Nursing by accredited program
  • Registered Nurse License recognized by the State of Michigan
  • Minimum of 1 year experience working as a Registered Nurse
  • Demonstrates knowledge and skills necessary to provide care to and communicate with the adult and geriatric populations
  • Demonstrates knowledge of the principles of growth and development over the life span
  • Able to assess data reflecting the client's status and interpret the appropriate information needed to identify each client's requirements relative to their age-specific needs
  • Demonstrates computer skills sufficient to use the electronic medical record effectively
  • Valid Driver's License
  • Current Auto Insurance
  • Reliable Transportation

Hospice RN Responsibilities

  • Completes an initial , comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Writes and initiates plan of care, regularly re-evaluates plan and patient/family/caregiver needs.
  • Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventative and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physicians in the physician's plan of care.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Assists the patient with activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
  • Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
