Social Worker Tech/Discharge Planner

Location: Evergreen - 19933 W. 13 Mile, Southfield, MI, 48076

Skills: The Discharge Planner is a member of the interdisciplinary team within the Behavioral Medicine, Social Work team. Responsible for patient advocacy, referrals, and discharge planning for patients and families. Makes reports, communicates with external agencies as required, participates in the discharge planning process, aids in implementing treatment goals, following policies and procedures.

Responsibilities

• Responsible for patient advocacy, referrals, and discharge planning for patients and families
• Makes reports, communicates with external agencies as required, participates in the discharge planning process, aids in implementing treatment goals, following policies and procedures
• Works to meet the discharge planning requirements for all patients determined to be in need of those services
• Must understand and demonstrate an ability to provide discharge planning and works together with Case Management and Physicians in respect to DRGs and utilization review
• Participates in overall quality improvement plan and departmental meetings/updates

Qualifications

• Associate or Bachelor's Degree in Social Work, Psychology, Sociology, or related Human Services field required
• One year of acute care or social agency experience in order to gain necessary skills and knowledge of discharge planning and community resources
