Optalis Healthcare & Rehabilitation is seeking a Social Worker for The Riverview Care Center in Columbus, OH.

Our Social Workers are part of a team approach to patient care, and will assist with resident and family members' questions, make patient assessments, follow care plans, perform discharge procedures, and coordinate hospice and home care services.
