Join the Opportunity House team and make a difference in the lives of others! Caring and compassionate individuals are needed to work with clients with intellectual and developmental disabilities in our Community Supports program located in Sycamore, IL.

The Case Manager/QIDP at Opportunity House is invaluable to our mission to help people with developmental disabilities work, live in homes of their choice, and enjoy community life. Duties include but are not limited to:

Summary

Provide case management for assigned clients. Assist clients in their home and community.

Essential Functions

· Cooperate with the ISC to design and implement service plans for assigned clients.

· Help clients participate in the design of various community, residential, and vocational support services tailored to their needs, and help individuals access services specified in their plans.

· Monitor and report client progress while maintaining accurate records.

· Ensure that individual rights and responsibilities are known and met.

· Communicate regularly and cooperate with all interdisciplinary team members, other agencies and parents/guardians.

· Ensure the safety and health of clients by identifying and eliminating safety hazards, establishing and enforcing safety rules, and reporting accidents promptly.

· Comply with and enforce licensing and regulatory requirements, accreditation standards and OH policy and procedures.

· Assist in the training and orientation of new hires, including modules of the DSP curriculum and OJT competencies.

· Arrange for transportation when necessary.

· Make recommendations for the budget and capital purchases.

· Maintain QIDP status, including Continuing Education Requirements.

· Other duties as assigned.

Knowledge, Skills, and Abilities

· Ability to analyze problems, develop solutions, and use independent judgment.

· Excellent organizational skills.

· Excellent written and verbal communication skills

· Ability to prioritize and perform duties without immediate direction or supervision.

· Ability to keep client and OH business information confidential.

· Skilled in Microsoft Office.

The Case Manager/QIDP must be a Direct Support Professional; DSP training is provided online and is flexible and paid. To become the Case Manager/QIDP, there will be additional training required. Opportunity House will provide you with the necessary training to succeed!

Required Education and Experience

· Bachelor’s Degree

· QIDP Qualified.

· At least one year of experience working with individuals with developmental disabilities

· Valid driver’s license and auto insurance
