OhioGuidestone is looking to welcome passionate and dedicated individuals to provide care coordination services to children and young adults in our Northeast Ohio area! The Care Coordinator will provide case management services to assigned clients (up to age 21) with severe and persistent mental health issues. This position is full-time. Start and End times for the role may vary. A typical day may begin anywhere from 8am – 11am and end between 4 pm – 7pm. The ideal candidate must be willing to be flexible and willing to adjust their hours to meet community needs. The training Schedule will be mostly in office during the first 6 months of hire.
