SUMMARY DESCRIPTION OF WORK

The Heartland Domestic Violence / Sexual Assault program provides several different services to survivors of domestic and sexual assault in an effort to meet them where they are at in their recovery journey. The program’s services include emergency shelter, advocacy, therapy, and other support services.

This is a housing first program and utilizes the progressive engagement model in providing services. This Case Manager III will work to screen and serve eligible clients residing at the Safe Haven shelter. This position will also work in close collaboration with other HFS programs, the Metro Area Continuum of Care for the Homeless, and other community programs, and resources, to ensure that employee’s and survivors have the necessary supports and best chance at success.

Compensation range: between $19.74 and $23.54 per hour (based upon number of years of experience)
Work Schedule: 24/7 facility, days/hours vary, including weekends/holidays (25 hours per week), shifts and hours to be discussed during interview.

Essential Duties and Responsibilities

  • *Responds to hotline calls, assess safety, provides safety planning and emotional support, maintaining strict confidentiality.
  • *Conducts interviews and assessment to accept, deny, and make appropriate and accurate referrals to hotline callers requesting emergency shelter.
  • *Accurately and thoroughly complete all required paperwork and data entry per agency policies and funding guidelines and within prescribed timelines.
  • *Completes all required monthly, quarterly, and annual reports for quality improvement and grant purposes.
  • *Assists the clients in identification of strengths, needs and goals. Develops an individualized case plan in cooperation with the client. Monitor and evaluate program participant progress.
  • *Provides problem solving and crisis intervention services to empower clients to reach self-sufficiency.
  • *Demonstrates knowledge of resources that are available to survivors of domestic and sexual assault and interested in engaging in training opportunities in order to increase knowledge to meet client needs.
  • *Demonstrates ability to help clients effectively by referring to and utilizing agency and community resources to obtain self-sufficiency.
  • *Develops personal and program related skills through participation in internal and external training opportunities including printed material and audio and/or visual media
  • *Attends trainings and meets minimum agency and program training requirements.
  • *Facilitates education and support groups to program participants as assigned.
  • *Facilitates education to community and community partners as assigned.
  • *Ensure safety and security measures for staff and residents are followed.
  • *Able to establish regularity and dependability in scheduling all appointments and accuracy in reports and statistics.
  • *Can work both independently, with little supervision, and as a part of a team.
  • *Assists with training incoming staff and volunteers.
  • *In town travel is required, includes use of personal vehicle.
  • *Participate in weekly team meetings with HFS program staff.
  • *Recognize and report maintenance and upkeep issues to designated staff.
  • *Completes housekeeping tasks as assigned and/or needed.
  • *Strives to make connections between the agency and the larger community whenever possible in order to contribute to the agency’s ongoing fundraising and friend-raising efforts.
  • *Creates, maintains, and shares, as appropriate, a dynamic self-care plan.
  • *Abides by all specific program and Agency policies, procedures, and requirements.
  • *Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the Agency.
  • *Develops positive working relationships with clients, referral sources, service agencies, and others encountered during work.
  • *Demonstrates the agency’s core competencies, safety, trustworthiness, choice, collaboration, and empowerment.
  • *Performs other program related duties as assigned.

PREFFERED MINIMUM QUALIFICATIONS

  • Bachelor’s Degree in Social Work or Human Service field, and two years direct care experience in Human Services preferred; or equivalent 
  • Valid Driver’s License and acceptable driving record
  • Proficient in speaking, reading, and writing English and Spanish preferred
