SUMMARY DESCRIPTION OF PROGRAM
Heartland Bridges houses two programs, Bridges Transitional Living and Bridges Crisis Care. Transitional Living is a short-term transitional housing program enriched with supportive services designed to assist people with a diagnosed mental health or co-occurring condition while they transition to stability in the community. Crisis Care is a residential stabilization service designed for individuals who need a safe, secure environment for 3-5 days while stabilizing from a crisis before returning to the community where they live.
SUMMARY DESCRIPTION OF WORK
The Case Manager I serves as a direct care staff that provides intensive case management to Bridges transitional living program participants. This role coordinates supportive services designed to meet the needs of the client while working with them to develop a plan for permanent housing. Decision making, problem solving, and highly developed interpersonal skills are critical.
Compensation: between $15.29 and $18.23 per hour (wage is based upon years of experience)
Work Schedule: overnight shifts, (10p.m. to 8:00 a.m.), 40 hours per week
