The Staff Nurse will work within our Assertive Community Treatment program. This position requires a nurse with excellent communication and organization skills as well as attention to detail.
This member of the ACT multidisciplinary team Is responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services and sharing shift-management responsibility with other staff.
Essential Functions
Duties:
  • Coordinate the medical portion of client placement while at OhioGuidestone
  • Update and maintain client charts as needed
  • Complete nursing assessments for clients
  • Provide diagnostic tests, as needed
  • Make appointments for clients to see health care professionals
  • Prepare client medical reports on a quarterly basis
  • Establish and maintain an open line of communication with all therapists, program managers, and off campus programs
  • Concurrently document of pharmacological management interactions
  • Maintain a minimum productivity level of 50% billable face-to-face contacts per week in an in-home setting
  • Maintain a high quality of treatment with the ACT team as measured by outcome measures, no-show rates, productivity, client surveys, supervisory evaluation.
  • Instruct staff regarding medications and medical procedures for each client
  • Provides ongoing education for infectious disease, Hepatitis vitals, and medications for staff
Psychiatrist related:
  • Meet weekly with psychiatrist(s), therapist(s), cottage manager(s), and client(s)
  • Prepare psychiatric note sheets
  • Carry out orders as given from doctors
  • Monitors height, weight, blood pressure, and pulse of clients and reports to psychiatrist
  • Attend weekly nursing meetings
  • Rotate shift as needed
Performance Requirements:
  • Must be able to clearly and confidentially communicate with OhioGuidestone employee, clients, and other stakeholders
  • Must be able to verbally exchange information in a confidential manner with clients and/or families in various settings regarding treatment plans and performance of client
  • Must be able to operate a computer and other position related technologies
  • Must be available for crisis management by phone and work flexible hours
  • Must be able to drive between OhioGuidestone work locations
  • Driving may also include transportation of clients
  • Must maintain regular and reliable attendance to the satisfaction of management
Physical Requirements:
  • Must be able to remain in a stationary position 50% of the time for desk work
  • Must be able to constantly position self to an equal level in order to work with children and/or clients of all ages
  • Must be able to run and/or chase as needed
  • Must be able to walk to and from cottages on campus
  • Must be able to frequently drive to varying work environments including agency office, school based, community based including client homes in various communities
Qualifications:
  • Associate’s Degree required; Bachelor’s Degree preferred
  • Valid State of Ohio Registered Nurse License required
  • Valid Driver’s License with safe driving record according to agency policy and proof of auto insurance required
  • Ability to take and pass a physical exam, tuberculosis test, and drug screening required
