Compensation:

$83,000-$113,000 - Commensurate with experience

$10,000 - Sign-on Bonus - (payable $5k at 6 months, $5k at 1st yr anniversary)

Responsibilities

• Administers treatments and/or medication as ordered by the physician.
• Visits each residence according to prescribed schedule to evaluate the individuals’ physical conditions.
• Reviews all health care provided to individuals including, related materials, recording notes and findings in the electronic medical record system and in the health care records.
• Participates in interdisciplinary team meetings.
• Acts as a liaison between the Individuals and the providers of health care in the community as well as with the consumer’s family and with other professional and para-professional staff.
• Performs assessments, problem identification & resolution and evaluation of the consumer’s health care needs.
•  Agree to follow federal, state and local governmental regulatory guidelines pertaining to a safe, healthy and clean work environment, reporting health and safety concerns to the supervisor.
• Complete all tasks deemed necessary to provide optimal health care to Opengate Individuals.
• Performs other duties as assigned

Qualifications

• Must have RN License in New York State
• Must have Drivers License in Tri-State Area (NY,NJ,CT)
• (1) year experience working with people with Intellectual and Developmental Disabilities
• Technical expertise in rendering nursing care
• Ability to work a flexible schedule.
