Shared Living Provider

Open Sky Community Services is seeking a Shared Living Provider in the Gardner area for man in his eighties. This individual enjoys attending his day program, going out into the community, and finding ways to stay active. He needs assistance with the majority of ADLs, medication management, diabetes management, and coping skills. He wears hearing aids and needs frequent reminders to slow down as his mobility is on the decline. He does well in a supportive and patient environment. Minimal stairs and a first-floor bedroom/full bathroom needed. Clinical experience preferred. Homes with older children will be considered.
