OPEN HEARTS is Hiring a Medical Wellness Liaison - Bilingual Near Phoenix, AZ

Essential Functions (duties):

Direct Duties:

  • Assist medical staff (NP and MD/DO) with all vitals for all psychiatric appointments.
  • Complete all reminder calls for upcoming psych appointments within 24 hours of appointment.
  • Manage all pharmacy requests for refills, Prior Authorizations, and coordinate with medical staff to fill requests.
  • Assist medical staff with collection of health information as outlined by RBHA.
  • Complete all documentation within 48 hours, at a minimum of 90% of the time.
  • As needed perform as agency receptionist, greeting all visitors and answering the phone in a pleasant, positive and professional manner that reflects agency confidentiality and security policies;
  • Document in Credible all contact with pharmacies, RBHA and consumers regarding medication requests, prior authorizations and psychiatric appointments.
  • Perform case management functions as directed by the agency psychiatric providers within 48 hours of request.
  • Document no shows, cancellations, and rescheduling of psychiatric appointments. Ensure follow up outreach is conducted for all no show appointments within timeframes dictated by ADHS.
  • Provide translation services as needed for psychiatric appointments

Administrative Duties:

  • Maintain PCP notifications for psychiatric providers as outlined by RBHA and attach to all consumers chart in Credible system.
  • Assist medical providers in coordinating records requests for outside stakeholders, i.e. DES, schools, PCP offices.
  • Ensure all documentation is scanned and attached to designated charts in the timeframes.
  • Document the offering of emergent psychiatric evaluation appointments to families.
  • Complete all required documentation within 48 hours.
  • Assist in maintaining psychiatric provider schedules, as the main point of contact for providers schedule requests.
  • Acts as the main point of contact to assist with medication issues, pharmacy request, etc with providers when providers are offsite.
  • Lock and secure the building at the close of the business day; or ensure staff members remaining on the premises after business hours are able to perform such duties
  • Practice and adhere to the "Code of Conduct" philosophy.
  • Assist in actively assessing work area for non-compliance issues and notify supervisor in a timely manner to ensure follow up.
  • Complete all required documentation and billing requirements accurately and in the assigned time frames
  • All other duties as assigned.

Minimum Education and Experience:

  • Medical Assistant certification preferred; preference given to experience within behavioral health systems.
  • Must possess knowledge and use of computerized software programs, including demonstrated proficiency with Microsoft Office applications including Office, Excel, Access, Publisher and Outlook.
  • Must be bilingual.

Prerequisites:

  • Must be 21 years of age.
  • Possess an Arizona driver’s license and state mandated auto insurance.
  • Possess or be able to obtain a Level One Fingerprint Clearance Card.
  • Must be able to pass a 5-panel drug screen
