Position Overview:

As a member of our team, your primary responsibilities will include providing skilled nursing care to clients of all ages in their homes, coordinating care with the interdisciplinary team, patients, families, and referring agencies, and assuming overall responsibility for care coordination.

Key Responsibilities:

- Conduct comprehensive assessments of patients' physical, emotional, social, and environmental needs, including risk assessment for grief and consideration of cultural and spiritual factors.

- Develop and modify individualized plans of care as necessary.

- Educate patients and families on disease processes, self-care techniques, dietary needs, and end-of-life care, and provide training to other staff members as needed.

- Implement preventive and rehabilitative nursing procedures as appropriate.

- Maintain detailed clinical and progress notes to track progress towards established goals.

- Coordinate patient and family services in collaboration with the interdisciplinary team.

- Utilize case management approach and make referrals to other services as required.

- Communicate effectively with physicians and other team members regarding changes in patient needs and response to care.

- Assess caregiver capabilities and provide support as needed.

- Participate in on-call duties as assigned.

Qualifications:

- Graduation from an accredited Diploma, Associate, or Baccalaureate School of Nursing.

- Current state licensure as a Registered Nurse.

- Preferred two years of experience as a Registered Nurse in a clinical care setting, with hospice or home health experience being advantageous.

- Proficiency in nursing skills adhering to accepted standards of practice.

- Excellent interpersonal and communication skills.

- Strong decision-making abilities.

- Proficiency in written and spoken English.

- Valid state driver's license.
