ONHL Home Care Services is currently looking for an ADvantage Case Manager.

• Certified ADvantage Case Management certification is preferred but not required.
• Experience with the OKDHS Harmony System is also preferred but not required.
• Time management, organization, communication, and decision making skills are a must.
• Full time field position with no on call or weekends.
• The position has autonomy and the Case Manager works primarily from home with a minimum of one office day per week required.
• Willingness to travel is required and travel reimbursement is provided.

Requirements

• LPN or Baccalaureate degree with one year paid professional experience.
• One year paid professional experience MUST BE with aging, senior, or disabled population, performing duties which encompass the core functions of case management. The core functions of case management include: assessment, planning, implementation, monitoring and reassessment.
