Job Description: As a Registered Nurse Case Manager at OneWell, you will play a crucial role in providing both direct and indirect patient care using your professional nursing skills and knowledge. You will be responsible for analyzing the health status of patients, determining nursing care needs, and developing patient care plans. This role involves overseeing a caseload and collaborating with a team of Licensed Practical Nurses (LPNs), Registered Nurses (RNs), Certified Nursing Assistants (CNAs), and Home Health Aides (HHAs) within designated territories.

Key Responsibilities:

  • Evaluate the effectiveness of nursing care provided and make necessary adjustments.
  • Participate in the development of individualized care plans based on age, cultural, and spiritual values.
  • Resolve client complaints and problems in a timely and professional manner
  • Maintain regular communication with management, providing status and activity reports as agreed.
  • Complete all required reports and documentation.
  • Conduct in-home visits, patient evaluations, and nurse evaluations (25% travel required)

Experience and Qualifications:

  • Nursing Experience: 3 years (Required)
  • Prior In-Home Case Management Experience: 1 year (Preferred)
  • Registered Nursing [RN] License (Required)
  • Bachelor of Science in Nursing [BSN] (Preferred)
  • Previous DDSN and DHHS experience (Preferred)

Expected Compensation: $34/hr-$39/hr ($70,720-$81,120/year); negotiable

Hours of Operation: Monday-Friday: 8:00a-4:30p (40 hours per week)

Reporting Structure: The Registered Nurse Case Manager will report to the IDD, Home Health, and Nursing Executive Supervisor.
