One80 Place is Hiring a Veteran Housing Case Manager Near Charleston, SC

Description

The work of the Housing Case Manager involves orienting all eligible Supportive Services for Veteran & Families participants to the program and providing housing search and supportive services to promote participants self-sufficiency, integration into the community, and permanency in housing; performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed both in a general office environment and in the field and community where program participants reside. 

The Housing Case Manager position is funded in whole by the VA through One80 Place’s SSVF Program. 

The Housing Case Manager will dedicate 100% of their time on Veterans and their families served by the SSVF Program. One80 Place ends and prevents homelessness throughout South Carolina with offices located in Charleston and Columbia. This position will be based in Charleston.

Requirements

SPECIFIC REQUIREMENTS:

1. Supports and demonstrates One80 Place’s mission, vision, and values. 

2. Belief and ability to demonstrate Housing First philosophy and strategies.

3. Bachelor’s degree required, preferably in Human Services. Experience may be considered in lieu of a degree.

4. Minimum two (2) years’ experience in case management.

5. Excellent verbal and written communication skills. Excellent computer skills including basic office programs and the ability to learn and utilize the HMIS database.

6. Ability to work independently and as part of a fast-paced team.

7. Ability to relate effectively to diverse individuals.

8. Data entry and basic skills for use of Excel, Word, and Outlook.

9 . Willing to travel throughout the the 7 county LHC service area.

10. The ability to maintain electronic client files. 

11. Excellent attention to detail with administrative task. 

SPECIFIC DUTIES:

Rapid Rehousing Activities 

1. Identify, target and engage clients begin discussions about permanent housing. 

2. Facilitate the housing planning process, by completing the housing barrier assessment, housing preference worksheet, and other necessary intake forms. 

3. Assist Veterans in obtaining necessary documentation for both housing and employment.

4. Assist Veterans in locating and securing housing of their choice.

5. Identify participant strengths and barriers to stability and assist participants to reducing barriers and linking to resources and services.

6. Ability to pay close attention to detail on administrative forms for the purposes of enrollment and determining eligibility for SSVF and administering financial assistance. 

7. Administer temporary financial assistance (TFA) by gathering the proper documentation for submission to finance in NXT.

Housing Stability Case Planning

1. In collaboration with Veteran household, develop housing stability plan addressing crisis housing needs, obtaining and maintaining permanent housing, participant goals, actions steps, case manager interventions, and referrals. 

2. Assist households in understanding the importance of following through on all expectations of their lease, including on-time rental payments, caring for their unit, etc. 

3. Update progress, goals, actions, and interventions as needed.

Strength-Based & Housing First Case Management 

1. Provide strengths-based case management and supportive services to Veterans households.

2. Conduct office and in-home visits with Veterans per the housing stability plan. 

3. Document progress, linkages and referrals to permanent housing, mainstream, Veteran-serving, and community-based resources, including income supports.

4. Be proactive in your approach to case management and approach every housing opportunity with a sense of urgency.

5. Ability to have diversion conversations to encourage clients to use support networks they may not think are a viable option to end their homelessness. 

Housing Counseling

1. Assist Veterans to create a budget and to identity safe, affordable housing considering Veteran preferences/income. 

2. Assist Veterans in identifying realistic housing options.

Teamwork and Collaboration:

1. Works well in collaboration with all other One80 Place staff to facilitate a team environment.

2. Participates in CES with the appropriate CoC and other partner agencies.

3. Participates in team discussions regarding client progress or lack of progress, with possible solutions to ensure best support for success. 

4. Actively participates in weekly housing team meetings and commits to group decisions.

5. Attends scheduled training programs for professional development.

6. Role models effective team behavior.

7. Demonstrates effective communication skills in building relationships with all One80 Place employees, volunteers, vendors, Board of Director’s, clients and guests.

8. Reports to work on time, well-groomed, appropriately dresses and ready to serve as a positive role model to all clients.

9. Substitutes for other case management staff when the need arises.

10. Excellent at collaboration with landlords, VA Staff, and other community organizations to best serve clients with the necessary community supports to successfully maintain permanent housing after SSVF services end. 

11. Ability to problem solve independently or in collaboration with peers. 

PHYSICAL, ENVIRONMENTAL AND SENSORY DEMANDS:

1. Corrective vision and hearing to normal range.

2. Ability to move between service locations.

3. Ability to lift 50 pounds and to assist physically disabled clients with basic functions. 

4. Possible exposure to communicable diseases and vicarious trauma.
