Qualifications

RN first assistants must have an associate or bachelor of science degree in nursing or a diploma from a hospital-affiliated nursing school, and be licensed as registered nurses. Most also complete a registered nurse first assistant training program. These short-term programs, such as the one offered by Lorain County Community College, teach students everything from prepping patients to controlling bleeding. Nurses usually work their way up to the first assistant position, starting as scrub nurses or circulating nurses in the operating room. First assistant candidates should have at least two years of recent perioperative nursing experience and certification in perioperative nursing.

Surgical Duties

RN first assistants play a hands-on role in surgical procedures by controlling bleeding, suturing incisions and wounds, and intervening if complications arise. For example, if a patient’s blood pressure begins to drop during surgery, they might administer life-saving drugs to stabilize it. If a patient’s heart stops, they might perform CPR. They also monitor a patient’s vital signs, including heart rate, respiration, and pulse. If the patient begins to struggle, the first assistant alerts the physician so he can address the complications.

Patient Evaluation and Education

RNFAs also care for patients before and after surgery. Before a procedure, they meet with patients and family members to answer their questions about the surgery, anesthesia, and recovery. They also assess the patient’s physical condition to identify problems that would make surgery too risky. Following the surgery, they evaluate the patient’s condition again to determine if he’s ready for transport to the recovery room. They look at his vital signs and ensure everything is sutured and there’s no bleeding or other complications.

Working Conditions

RN first assistants work in a high-stress environment where they must think quickly and stay calm under pressure. Because they work so closely with the surgeon and fellow perioperative nurses, they must also have excellent communication skills and be able not only to follow the surgeon’s instructions but also communicate clearly to other members of the surgical team. In addition, they must follow strict safety protocols to protect themselves and patients. With some surgeries lasting several hours, they often spend a lot of time on their feet.
