POSITION SUMMARY

Primary functions are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the Interdisciplinary Group/Team, patient/family, and referring agency; and assume the responsibility for coordination of care.

POSITION DUTIES AND RESPONSIBILITIES

  • Patient Assessment and Care Planning:
  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patients physical, functional, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, evaluations, and ADLs, (i.e., risk for grief, cultural and spiritual, verbal, and non-verbal).
  • Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs.
  • Develop individualized care plans in collaboration with the patient, their family, and the hospice team.
  • Continuously update and adjust care plans based on the changing needs and conditions of patients.
  • Assessment Using Nursing and LCD Criteria:
  • Utilize the Nursing Assessment and Local Coverage Determination (LCD) criteria to identify physical, psycho-social, and environmental needs of patients.
  • Infection Control:
  • Implement and adhere to OSHA standards and infection control measures to protect patients and staff.
  • Symptom Management:
  • Administer medications and treatments to manage pain, discomfort, or other symptoms associated with terminal illnesses.
  • Monitor and assess the effectiveness of pain and symptom management interventions.
  • Collaboration with the Hospice Team:
  • Work closely with the interdisciplinary hospice team, including doctors, social workers, chaplains, therapists, and volunteers, to ensure holistic care.
  • Participate in regular team meetings to discuss patients' status, needs, and care plans.
  • Documentation and Reporting:
  • Maintain accurate and comprehensive patient records.
  • Document assessments, interventions, medications, patient responses, and any changes in condition.
  • Report significant changes in a patient's condition to the attending physician and hospice team.
  • Conduct an initial assessment to determine the patient's eligibility and appropriateness for hospice care.
  • Gather information on the patient's medical history, current condition, medications, and specific needs.
  • Assess family dynamics and support structures to understand the care environment.
  • Ensure timely submission of all required documentation, reports, and paperwork to meet regulatory and agency requirements.
  • Family Support and Education:
  • Assist families in understanding the dying process and what to expect.
  • Provide resources and referrals for family members, such as counseling services or support groups.
  • Team Leadership and Direction
  • Offer clinical guidance to both the Hospice Aide and LPN/LVN, ensuring that service provision is of the highest quality and continuity.
  • Admission Assessment (If Applicable):
  • Conduct an initial assessment to determine the patient's eligibility and appropriateness for hospice care.
  • Gather information on the patient's medical history, current condition, medications, and specific needs.
  • Assess family dynamics and support structures to understand the care environment.
  • QAPI (Quality Assessment and Performance Improvement):
  • Participate in quality assessment processes, ensuring continuous improvement in care delivery.
  • Report any areas of concern or potential improvements to the hospice agency management.
  • Understanding Hospice Philosophy:
  • Embrace and promote the hospice philosophy and ensure care is aligned with administrative policies.
  • On-call Responsibilities:
  • Participate in the on-call rotation, providing after-hours care and support as needed.
  • Communication:
  • Maintain open communication with patients, families, and the hospice team.
  • Provide updates on patient status, address concerns, and answer questions in a compassionate manner.

POSITION SKILLS AND QUALIFICATION

Required:

  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Current State license as a Registered Nurse; Current driver’s license.
  • Two (2) year’s experience as a Registered Nurse in a clinical care setting and one (1) year full time hospice.
  • Should be CPR certified.
  • Reliable transportation and valid auto liability insurance.
  • Familiar with electronic health records documentation software.
  • Must be able to read, write, and comprehend English.

Preferred:

  • Electronic Health Records (EHR) or Electronic Medical Records (EMR) Systems:
  • Ability to navigate, document, and retrieve patient information from systems such as WellSky or Consolo
  • Microsoft Office Suite Proficiency: Word and Excel.
  • Communication and Collaboration Tools:
  • Email platforms
  • Virtual meeting software: Zoom or Microsoft Teams for remote meetings, especially important if coordinating with external healthcare providers or families.
