Responsible to: Clinical Services Supervisor Job Summary: This position will support and guide children and families throughout the evaluation and treatment process. They will work closely with children, families, center staff and community resources. Essential Job Duties:-Support families by serving as a point of contact for families and promptly responding to and communicating their needs or concerns; especially during the evaluation process-Support families through the Office for People With Developmental Disabilities - Front Door intake process as needed- Assess eligibility for and assist families with enrollment as needed.-Provide in-out reach to families on the waitlist and those currently in the evaluation process or those already receiving services. This could include, but not limited to assistance completing paperwork for intake, providing knowledge about available services and assisting families with obtaining such services, providing linkage and referral to community resources for families served by the Evaluation and Treatment Center (ETC). -Participate as a member of ETC’s multidisciplinary team. -Help families work with insurance companies to gain benefits and/or access to services -Use case management and clinical skills to help patients and families address and resolve social, financial, and psychological problems related to the patient's health-Provide psycho-educational presentations for the patients and families in the ETC -Initiate and maintain appropriate documentation of interaction with or on behalf of clients in the form of the social history and assessment, routine care meetings, and progress notes-Any other duty as assigned by your Supervisor. Skills: -Natural ability to connect with children and parents-Good time management -Excellent written and verbal communication skills Minimum Education Required: - Bachelor’s degree in social work or related fieldMinimum Experience Required: -At least 3 years’ experience working with children and families -Experience with OPWDD service delivery preferred License/Registration/ Certifications Required:-NYS Drivers licenseTraining Requirements: -New Hire OrientationPhysical Requirements: -The constant physical demand of the position is sitting, standing, walking, talking, hearing, vision to be performed with or without reasonable accommodation. Employees need to be able to get on the floor and play.-The frequent physical demands of the position are, repetitive motions, and eye/hand/foot coordination to be performed with or without reasonable accommodation.-The occasional physical demands of the position are lifting, carrying, climbing, pushing, pulling, balancing, stooping, crouching, reaching, handling, grasping, and feeling to be performed with or without reasonable accommodation.
