We are currently seeking A Wound Care Nurse Practitioner to provide medical services to patients living in long-term care facilities.

NP WOUND CARE ROLE AND RESPONSIBILITIES:

· Provide wound care for long-term care patients residing in skilled nursing facilities.

· Prescribe appropriate medications.

· Order, review, and/or follow up with treatments and diagnostic testing.

· Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.

· and/or the care plan.

· Collaborate with primary care physician.

· Provider will assess and monitor residents on a regular basis.

· Provider will be available to assist staff in assessing wound and pressure ulcer risks.

· Provider will evaluate multiple factors related to wounds: i.e.

· Nutrition, Mobility, Incontinence associated dermatitis, Pressure ulcers, Diabetic ulcers, Arterial ulcers, Venous ulcers, and Common dermatologic conditions

MANAGEMENT

· Measures to decrease the risk of wounds with recommendations to staff.

· Measures to optimize healing via protocols

· Superficial wound debridement

· Dressing selection and management

· Placement of negative pressure wound dressings

· Post myocutaneous flap surgery care

NP QUALIFICATIONS AND EDUCATION REQUIREMENTS

· Current, unrestricted NP license in the state of New York.

· Board Certification.

· DEA licensure.

· Clinical background in wound care.

· Experience working in a long-term care setting.

· Ability to work with an EMR.

· Ability to manage patients with advanced illness and multiple chronic conditions.

· Ability to work within an interdisciplinary care team to coordinate patient care.

· Wound care experience

Job Type: Part-time

Pay: $495.00 - $500.00 per day

Expected hours: No less than 16 per week
