About CommuniCare OLE
Established in 2023, CommuniCare OLE is the result of a union of two health centers with a deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Nurse Manager is responsible for ensuring high quality nursing support, programs and services are provided by the nursing team to patients of CommuniCare OLE. The Nurse Manager directly reports to the Director of Nursing and works directly with all department supervisors and nurses to ensure adequate nursing coverage and optimal care. The Nurse Manager directly supervises the Registered Nurses (RN’s) which includes oversight of professional development and nursing competencies. The RN manager also works with MAs and Ops Supervisors to ensure that all nursing and MA staff are working within the top of each of their professions. The team of RN's are spread out in different clinics, which requires the Nurse Manager to travel to every site.

Knowledge of programs, insurances and access: eligibility guidelines and applications (including Medi-cal, CDP, CHDP, CPSP, Presumptive Eligibility, Family Pact, Sliding Scale) |
DUTIES AND RESPONSIBILITIES
Manages all Registered Nurses (RN’s) at all health centers, including call triage nurses and backline support nurses, DME staff and supports the vaccination programs of CommuniCare OLE. |
Provides direct RN nursing support in all care teams at all health centers as needed and ensures adequate cross training so that RNs may cover all RN responsibilities as needed |
Recruits, trains and orients RN’s for pod nursing, administrative support and complex care management and ensures proper RN staffing to meet clinical needs and to support the medical provider team |
Oversees the RN’s work schedules on a daily, weekly and monthly basis to ensure proper workflow and reviews, approves/denies requests for vacation or continuing education of the RN’s |
Schedules and facilitates weekly staff meetings and maintains accurate records for all meetings (including sign-in sheet) and storing in a place accessible to all staff |
Schedules in-services for RNs have to maximize clinical nursing skills and monitors CEU activity to maximize clinical nursing skills |
Constantly assess staffing, assigns and balances nursing staff where needs are to meet patient care |
Resolves conflicts in a timely and professional manner; utilizes verbal and written communication that enhances team effort and growth and evaluates, counsels and promotes professional growth and development of RNs; |
Assures nursing staff complies with safety and health policies and procedures of the health center; reviews infection control procedures and ensures they are in place. |
Schedules, trains and supervises Nursing student rotation or clinical experiences at CommuniCare OLE |
Develops policies, procedures, and workflows. |
Attends staff meetings and management team meetings and contributes actively to discussions |
Actively support QI initiatives and embeds nursing in all clinical workflows as appropriate |
Responsible in providing in-service and training to staff where appropriate; creates an environment of collaboration between MAs and RNs where work coordination requires a more team-oriented approach |
Teaches and mentors MAs on skills they need in order to elevate their ability to support patient care and reduce reliance on nurses on procedures that are within the MAs role and scope of work |
Ensures all staff are meeting the vaccination/immunization requirements and N-95 fit annually |
Understands and connects with the strategic priorities of the organization and bringing the nursing team into the care team as part of a cohesive patient-centered medical home model |
Meets regularly with different departments, teams, staff to develop goals, introduce new services, training purposes and introduction of new systems |
Has a strong advocacy for patient care, patients first principles and embodies these through the nursing workforce and workflows. |
Supports the vaccination/immunization program of CommuniCare OLE through staffing coverage, workflow coordination and training of staff and ensuring vaccine inventory is accurate and maintained |
Perform other duties as assigned |
