POSITION TITLE: Comprehensive Perinatal Services Program (CPSP) Supervisor 

DEPARTMENT: Perinatal

REPORTS TO: Perinatal Clinic Manager

LOCATION:  Davis, West Sacramento and Woodland

Established in 2023, CommuniCare OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. JOB SUMMARY: The CPSP Supervisor is responsible for supervising and enhancing the quality of the health education and CPSP services provided in the Perinatal (PN) Program. The CPSP Supervisor works closely with the PN Clinic Manager and the Associate PN Directors, under the direction of the CMO, to provide programmatic support to the prenatal clinics and enhance the quality of services provided by the PN program.

EDUCATION, EXPERIENCE, TRAINING

  1. Bachelor’s degree required. 
  2. Master’s degree in Community or Health Education or Public Health preferred.
  3. OR advanced clinic/medical experience/knowledge achieved through a combination of training and work experience.
  4. Experience working in a Women’s health care setting strongly preferred.
  5. Administrative and supervisory experience preferred.
  6. Bilingual English/Spanish preferred 

KNOWLEDGE OF WORK

1.

Demonstrated knowledge of local health care delivery system.

2.

Experience in prenatal and postpartum care 

BDUTIES AND RESPONSIBILITIES

1.

Travel to clinic sites in Davis, West Sacramento, Woodland 

2.

Hire, supervise, and evaluate Health Educators in the Perinatal Program. Ensure competency and adherence to CPSP guidelines. Ensure appropriate staffing of PN Health Educators to accommodate patient needs in the PN clinics. Help facilitate professional development of PN Health Educators.

3.

Manage monthly clinic schedule for Health Educator patient visits, CPSP provider patient visits (MSW, RD, Lactation and Perinatal RNs, and PN patient group and individual classes. Schedule management involves entering the schedule into the EHR, finding coverage for employee absences/vacations, and monitoring for needed adjustments based on patient census and staffing.

Manage and maintain CCHC CPSP Program including adherence to current CPSP Guidelines and Steps to Take educational materials. Review and update CCHC CPSP Protocols annually. Communicate with the managed care plansregarding staffing, protocol, and programmatic changes and assist with their biannual CCHC CPSP site review.

5.

Perform Monthly chart audits for QI to ensure proper documentation and care plan development and adherence to protocols by Health Educators. May perform monthly Partnership QIP. 

6.

Works as a team with PN Clinic Manager to fill in for staff absences to keep PN clinics open and operational.Fill in for PN Health Educator appointments and classes as needed for vacation and sick coverage.

7.

Maintain car seat program (through dept budget and/or grant funding) and health educator trainings as car seat technicians. Attend Car seat coalition meeting quarterly. Ensure CCHC participation in county car seat events.

8.

Run monthly Relevant report to ensure patient compliance to trimester and postpartum reassessment visits and class attendance.

9.

Engage in community to promote CCHC PN services and to develop relationships with program donors including but not limited to Yolo County Diaper bank, Davis Linus Blanket group, Sacramento Baby Carrier group, Pioneer Elementary School blanket project and other community donors. 

10.

Develop PN health education class curriculum including class outlines, patient educational materials, and pre- and post-tests. Maintain patient attendance records for Medi-Cal and CPSP review.

11.

Conduct monthly Health Education and CPSP team meetings and assist staff in attending periodic trainings to maintain quality up-to-date services and health education. Perform patient surveys, as needed, to assess community needs and interests.

12.

Lead in the development and maintenance of Group Prenatal Care, when conditions allow this model of care to restart, at the three prenatal clinic sites. Responsibilities include staff training, class content, patient flow, staffing, billing, scheduling, and sustainability.

13.

Ensure accurate and timely documentation and billing of CPSP services. Investigate any errors or concerns and provide timely corrections or staff trainings.

14.

Attend monthly PN billing meetings, regular meetings with Associate Perinatal Directors, regular meetings with Perinatal Clinic Manager, site manager meetings, space management meetings, County CPSP Round Table meetings, and any other meetings as needed.

15.

Communicate effectively with Medi-Cal, Partnership and other insurers to assist patients with obtaining health care coverage and problem-solving insurance issues as needed.

16.

Other duties as assigned.
