POSITION TITLE: Behavioral Health Outreach Specialist, Elevate Youth

DEPARTMENT: Behavioral Health

REPORTS TO: Director of Behavioral Health, Youth and Family

JOB SUMMARY: The Behavioral Health Outreach Specialist, Elevate Youth represents CommuniCare OLE and is responsible for supporting outreach, coordination, and education within the agency and among LGBTQ communities within Yolo County.

EDUCATION, EXPERIENCE, TRAINING

  1. A minimum of 2 years of experience coordinating outreach events and activities preferred.
  2. Ability to effectively manage time, interpersonal relationships, resources and information.
  3. Strong computer skills and Microsoft Office knowledge (Word, Excel, Power Point, Outlook, etc.)
  4. A Valid California Driver’s License is required.
  5. Bachelor’s Degree or related experience in behavioral health field preferred. 
  6. Bilingual in Spanish preferred.
  7. Strong verbal and written communication skills

KNOWLEDGE OF WORK

1.

Knowledge of Federally Qualified Health Centers and key health center principles and guidelines

2.

Knowledge of community health screenings and health fair events

3.

Strong ability to balance team and individual responsibilities; collaborates with other departments within organization as well as external partners; supports goals and objectives of all involved. 

4.

Strong event management skills including budgeting, promotion and reporting

DUTIES AND RESPONSIBILITIES

1.

Create and maintain outreach presentations with up to data, and accurate information that is applicable to LGBTQ topics, as well as services offered at CommuniCare OLE, including gender affirming care.

2.

Support in the coordination and planning of LGBTQ Pride and LGBTQ social events, including Gender and Sexuality Affirming (GSA) groups on school campuses to support inclusivity and safe spaces for youth.

3.

Review, track and monitor the Elevate Queer Yolo workplan to ensure to support team in making progress towards goals and objectives.

4.

Support youth and their families in navigating and accessing services including mental health, substance use and gender affirming care.

5.

Ability to effectively manage time, interpersonal relationships, resources, and information.

6.

Coordinate and schedule on-demand LGBTQ youth presentations throughout Yolo County, including travel to in-person locations and technical troubleshooting for virtual presentations.

7.

Collaborate with other behavioral health programs (SUD services, School Services, Youth/Family Services, Wraparound) to identify opportunities for partnership between programs and increasing youth LGBTQ presentations.

8.

Represent CommuniCare OLE Elevate Queer Yolo programs at partnership meetings, summits, collaboratives, and coalitions.

9.

Maintain positive relationships with community partners and stakeholders.

10.

Participate in staff meetings, provide feedback, and troubleshoot barriers faced by clients. Assist with coordination of care offering input on client progress and the development of the program.

11.

Support youth/families in navigating mental health and being a part of the LGBT , as well as providing coordination of care and linkage to community resources and services.

12.

Attend schedule Learning Collaboratives and training/conferences as arranged.

13.

Adhere to proposed program outcome projected timeline for grant deliverables, and tracking.

14.

Collect and compile data and statistics as required by the grant.

15.

Complete necessary documentation and tracking in a timely manner.

16.

Responsible for remaining within established ethical standards, including those of confidentiality.

17.

Responsible for maintaining the standards as listed in the CommuniCare OLE Health Centers Personnel Policies and Procedures Manual, and any other standards which may apply to this position.

18.

Other projects/duties as assigned.
