Job Responsibilities: Registered Nurse Home Health Care
Clinical Care:
  • Patient Assessment: Conduct comprehensive assessments of patients’ physical, psychological, and social needs in their homes.
  • Care Planning: Develop individualized care plans in collaboration with patients, families, and healthcare providers.
  • Medical Administration: Administer medications, treatments, and procedures as prescribed by physicians.
  • Wound Care: Provide wound care and management, including dressing changes and monitoring for signs of infection.
  • Vital Signs Monitoring: Regularly monitor and record patients’ vital signs, including blood pressure, heart rate, and respiratory rate.
  • Symptom Management: Identify and manage symptoms of chronic and acute conditions, providing appropriate interventions and referrals.
Patient Education:
  • Health Education: Educate patients and their families on disease processes, medication management, and lifestyle changes to promote health and prevent complications.
  • Equipment Use: Instruct patients and caregivers on the proper use of medical equipment and assistive devices.
Coordination of Care:
  • Multidisciplinary Collaboration: Coordinate care with physicians, therapists, social workers, and other healthcare professionals to ensure comprehensive care.
  • Care Transitions: Facilitate smooth transitions between hospital, home, and other care settings, ensuring continuity of care.
  • Resource Referral: Refer patients and families to community resources and support services as needed.
Documentation and Compliance:
  • Documentation: Maintain accurate and timely documentation of patient care, assessments, and interventions in compliance with agency standards and regulatory requirements.
  • Regulatory Compliance: Ensure all nursing activities comply with local, state, and federal regulations and accreditation standards.
Professional Development:
  • Continuing Education: Participate in ongoing education and professional development activities to stay current with best practices in home health care.
  • Quality Improvement: Engage in quality improvement initiatives to enhance patient care and outcomes.
Support and Supervision:
  • Supervision: Supervise and mentor home health aides and other support staff, providing guidance and support as needed.

Qualifications:
  • Registered Nurse (RN) with an active license.
  • Extensive homecare experience, with a strong understanding of Medicare/Medicaid regulations.
