Old Colony Hospice is seeking a full-time day shift Registered Nurse Case Manager. The Hospice RN Case Manager coordinates hospice services for the assigned caseload of patients/caregivers collaborating with an interdisciplinary team (IDT) and the patient’s primary physician in order to maximize the comfort and health of patients, families, and caregivers regardless of the care setting, to promote continuity and adherence to the hospice plan of care.
ESSENTIAL DUTIES:
Qualifications:
- Licensed to practice as an RN in the Commonwealth of MA
- Driver's License (required)
- 1-Year Nursing Experience (preferred but not required)
- Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
- Must have knowledge and understanding of applicable federal, state, and local hospice, Medicare, Medicaid, HMO regulations, as well as HIPAA and other regulations protecting personal health information of patients.
- Must be able to maintain reliable attendance and be flexible.
