Position Overview: As a Home Health LPN at Ohman Family Living At Home you will collaborate with healthcare professionals, patients, and their families to develop comprehensive care plans that address their unique needs as they transition home from a skilled rehab stay or in order to assist them in remaining home with additional care services. Your expertise and coordination skills will be crucial in facilitating effective communication between medical professionals and families to provide continuity of care. Home Health LPN Responsibilities include but are not limited to:

• Provides services to patients in their homes in accordance with the State Nurse Practice Act, agency policy, and accepted professional standards of practice.
• Provides skilled nursing services to patients in their homes communicate/document observations and assessments with Nurse Managers and the Care Team
• Monitor patient progress during their episode of care plans and involving interdisciplinary teams to optimize outcomes.
• Provide emotional support and guidance to patients and families throughout their care
• Maintain accurate and organized documentation of all interactions, care plans, and updates in patient’s electronic records.

Home Health LPN Qualifications:

• LPN License from an accredited program
• Home Health experience
• Current driver's license, good driving record, and reliable transportation.
• Strong communication and interpersonal skills.
• Passionate, motivated, and professional.
• Strong understanding of medical terminology, patient care, and rehabilitation processes.
• Excellent communication and interpersonal skills
• Detail-oriented mindset with the ability to manage multiple tasks and deadline
• Compassionate and empathetic approach to address the emotional needs of patients and families throughout their care
