The Case Manager is responsible for supporting participants to become confident not only in a professional setting, but to empower themselves in their personal lives. This includes developing and implementing job readiness and leadership curriculum aligned with contextualized career pathways, academic curriculum, employability workshops, occupational certification training and linking participants to post-secondary education, and/or long-term occupational training tracks.

Essential Functions:

  • Delivers a robust curriculum including job readiness, soft skill development, academic remediation, and college and career readiness. Update and adapt program curriculum as needed to meet the needs of individual referrals.
  • Creates and implements curriculum for professional skill development such as resume/ cover letter/ references, interview preparation, professionalism, networking and career advancement.
  • Provide academic support such as, tutoring, credit recovery assistance and communication with the school Guidance Counselor/Administrators to implement the proper supports for grade matriculation and or graduation.
  • Assist client to identify both short term and long term academic/career goals and monitor their progress through detailed case management notes.
  • Develops and maintains partnerships with networks, educational institutions and employers to connect referrals to build social capital.
  • Monitors and maintains spreadsheets/ database of attendance, curriculum completion and academic achievements for contract and performance outcomes.
  • Develops strong community supports and collaborative partnerships. Maintain effective linkages with internal and external department staff, outside agency professionals, and other community organizations as it related to data management.
  • Works closely with supervisor to ensure that all new referrals are contacted with 24 hours of initial referral and works closely with parent/guardian to ensure all orientation documents are completed.
  • Conducts home visits, as appropriate, to assess needs and support re-engagement of participants.
  • Organizes and maintains written documentation for participants including case notes, timesheets, incentive and supportive service forms, and evaluative data.
  • Meets the minimum outcomes per contract requirement which will be reviewed during scheduled supervision and team meetings.
  • Works collaboratively with the funder, the Promise Team, Parole Officer/Probation and any other program partner.
  • Participates in the planning, organization and implementation of special events/department specific projects including job fairs, community fairs, community service learning projects and fundraising events.
  • Maintains the confidentiality of client or collateral interaction within Agency defined standards.
  • Participates in staff training, supervisory and staff meetings.
  • Completes all contract training requirements.
  • Maintains regular and reliable attendance.
  • Exhibits positivity, flexibility and a willingness to provide in service training and take on new responsibilities as requested or required.

Required Skills/Abilities

  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
  • Computer literacy skills, including Windows applications, Microsoft Office Suite, and basic database usage, as well as basic typing.
  • Must be able to exchange information with participants and/or families in various settings regarding program plans and performance of participant.
  • Organizational skills and attention to detail.

Performance/Physical Requirements

  • Work consist of walking, bending, stooping, ascending/descending stairs, and remaining in a stationary position 50% of the time, including positioning self to an equal level in order to assist children of all ages.
  • Work environment is based in the community and can include an agency office, school-based setting, child care facilities or participant’s homes.
  • Must be able to drive frequently due to work varying work environments including agency office, school based, community based locations which may include participant homes in various communities.
  • Works flexible hours, which can include days, evenings, and weekends, as desired to meet the needs of clients and the OhioGuidestone and is available for crisis management by phone.
  • Interacts verbally with coworkers, representatives from outside agencies, and families.

Qualifications

  • Must be at least 21 years of age and minimum educational level of a Bachelor’s degree in Social Work, Human Services, related field and has experience with at risk-youth.
  • Must have a valid Ohio Driver’s License with a safe driving record and valid insurance.
  • Must have reliable transportation and the ability to transport clients.
  • Ability to take and pass a physical exam, tuberculosis test and drug screening.
  • Employment is contingent upon clear results of a thorough background check.
