Location: Cleveland, OH, US/Akron, OH, US

This is a full-time position covering the NE Ohio area Monday-Friday with hours of 8:00am-5:00pm.

Pure Healthcare is looking for a creative and experienced leader to join our team as a Licensed Social Worker (LSW). You will work alongside other members of the Pure Healthcare and Tuesday Health clinical teams, providing a multi-disciplinary approach to care for our members with serious illness and ensuring the delivery of high-quality supportive care services to our patients and their families. The Pure Healthcare LSW will work remotely and do home visits.

As a Licensed Social Worker, you will:

• Support administration of multiple assessments and screening tools within the clinical model at different times throughout the care timeline
• Ability to interpret responses to assessments and screening tools and support prioritization of need based on responses
• Escalate appropriate responses, results, findings to appropriate clinicians following the escalation procedure
• Contribute content for care plan development within the multidisciplinary care team
• Support internal and joint rounds to ensure the member experience is optimal and coordinated
• Learn the electronic systems, tools, technology, to deliver a coordinated approach to ensure the members have a unique experience
• Build a strong, trusting relationship with every member, where listening and empathy are the foundation of every interaction
• Contribute to the multidisciplinary team transforming care for members with serious illness

To be successful in this role, you will need:

• Active and unrestricted LSW or LISW in the State of Ohio
• Experience in clinical/medical setting preferred 
• Experience in a multi- disciplinary care model working across network providers, health plan care management support, and employed clinicians
• Strong communication and organization skills
• Ability to multitask and work under pressure without sacrificing a positive member experience
• Ability to know when to escalate and potentially act professionally in an emergent situation
• Strong technical skills to navigate different documentation systems and tools; as well as ability to learn new systems 
• Strong written skills
• CPR Certified required
• Ability to drive during daytime, nighttime, or inclement weather. This position requires travel to different work locations and within the counties we serve
• Valid Driver's License with Safe Driving Record with successful clearance of a driving record check
• Successful clearance of a criminal background check
• State minimum automobile insurance coverage
