Ohel’s Project Hub@Home is seeking Case Management Supervisors who will be will be responsible for managing and motivating a team of Case Managers working with Older Adults living in the Community. The Case Management Supervisor will provide supervision to the staff, while ensuring compliance with standards and protocols required by NYC Aging for the initiative. 

Responsibilities:

• Provide weekly individual supervision and monthly group supervision to case management staff
• Support, and motivate team as well as provide education and training
• Monitor documentation and conduct review of monthly statistics for internal use and reports to NYC Aging
• Maintain and carry a small caseload as needed
• Represent Ohel at community outreach sessions, conferences, and other networking events as needed
• Assist with special events and projects

Qualifications:

• LMSW and three years of supervisory experience within the case management/ social work field required
• Extensive knowledge of community resources in Brooklyn
• An understanding of NYC Aging standards for Case Management
• Ability to work independently and collaboratively
• Bilingual Creole or Russian speaking preferred

The ideal candidate is a self-starter who can work independently as well as within a team and has excellent follow up skills. This Full time position is located in Brooklyn, offers a hybrid schedule, with home visits as needed. Position includes a competitive benefits package.

Salary: $70,000- $80,000
