• Work Dates Needed: ASAP-Ongoing 
• Preferred Schedule: Monday – Friday , 8a-5p 
• Worksite Setting: Outpatient 
• Scope of Work: Provide comprehensive medical services for adult members of family; no peds. 
• Services include primary care and treatment, Preventative care, Immunizations, health education, prescribing of medications, ordering, or interpreting lab testing, physical examinations, STD screenings, Vision/hearing, family planning, and nutrition. 
• 16-24 patients per day. 
• Licenses, Certifications, Requirements: CT License, DEA, BLS; BC, EPIC Experience Required EMR: Epic 
• Credentialing: 60-90 days; temps avail w clean file
• $500 referral bonus received for any referral who works 30 days.
• $Competitive Hourly pay housing travel A rated malpractice insurance with tail coverage included.
