Independent Living Coordinator - Nursing Home Transition Specialist
The IL Coordinator is responsible for assisting persons having disabilities, who currently reside in a nursing home facility and who prefer to live in the community to transition safely into the community setting.
Key Responsibilities
Reporting to the Program Manager, the IL coordinator's duties include outreach into nursing homes for referrals to the program, accepting referrals for transition, assessing for possibility of safe transition, and assisting with the orchestration of services to facilitate a safe transition into the community. Primary responsibilities include:
· Oversee the progression of all services as established in the consumer’s IL transition plan.
· Provide advocacy and other community-based referrals/services to address barriers to the above IL Transition Plan.
· Provide on-going support services to consumers involved in the nursing home transition program up to a year after transition has taken place. Assisting with direct application assistance, as appropriate.
· Provide education and outreach to nursing home staff as to the options of community living for those nursing home patients who are capable of living on their own with support services.
· Keep accurate, detailed, up-to-date records/reports of all consumers in this program, identifying progress, accomplishments and/or complications and barriers.
· Work as part of an interdisciplinary team with all direct service staff involved in this project.
· Assist with securing home housewares for consumers and completing POs as appropriate.
· Provide public outreach regarding services/awareness activities.
· Responsible for monthly/quarterly program reports as requested by Management.
· Other duties as assigned by the Program Manager
Education:
Bachelor’s Degree or higher from an accredited college or university in a human service-related field
Experience:
Three years’ experience in a position with similar responsibilities, five years’ experience preferred.
Salary: As determined by experience, credentials, and the financial consideration of OSCIL.
Hours: 35 hours per week
Performance Evaluation and Probationary Period: All new employees will be subject to a probationary period of six months. The supervisor shall perform bi-monthly written performance evaluations during the probationary period, with a performance evaluation being performed annually thereafter.
Job Type: Full-time
Pay: $40,000.00 - $43,680.00 per hour
