Ocean Healthcare is Hiring a Weekend LPN Patient Admissions Coordinator Near Hamilton, NJ

Serenity Hospice provides caring and compassionate hospice care services to patients and families throughout New Jersey. Our mission is to alleviate pain and symptoms; foster a state of calmness and harmony; and encourage an appreciation of family, friends, and life experiences.

Serenity Hospice Care is looking for an organized and compassionate Intake Coordinator to join our team! The Intake Coordinator will be responsible for working with hospice patients and their families, they discuss services and act as a liaison between facilities, marketing, social workers and nurses. The Intake Coordinator will report to the Director of Admissions and will relay all information as needed.

This position is located in the Serenity Hospice Care Inpatient Unit at RWJ Hamilton and will follow the following schedule: Friday through Sunday (9 AM to 9 PM)

Responsibilities:

Candidate must be available to train in the Bordentown office Monday through Friday from 8:30am to 4:30pm for up to 6 weeks.

  • Offer professional and positive interactions with clients and their families.
  • Process and document new referrals.
  • Verify Patient Insurance
  • Track census daily for assigned territory.
  • Track and document level of care for patients.
  • Work with staff to place patients in appropriate facilities.
  • Keep Director of Admissions informed of admissions taking place.
  • Contact referral sources as needed to receive updates on intakes.
  • Track documentation for compliance purposes.
  • Track documentation as needed for billing.
  • Provide weekly feedback status to the Admissions team.
  • Answer phones and filing as assigned.
  • Other duties as assigned.

Requirements:

Candidate must be available to train in the Bordentown office Monday through Friday from 8:30am to 4:30pm for up to 6 weeks.

  • MUST have a minimum of One Year Hospice Experience
  • Excellent communication skills - verbal and written.
  • Proficient skills in Microsoft Office.
  • Reliable transportation and valid drivers license.
  • Ability to multi-task and work under pressure.
  • Organized and self motivated.
