Must have an NJ Social Worker Certification or License in good standing. Aspen Hills Healthcare Center, a 204 bed sub-acute and long-term care facility located in Pemberton, NJ is seeking a Social Worker to join our team of professionals and to assist our Director of Social Services.

Duties include, but are not limited to:

* Admissions and Ongoing Assessments/Documentation

* Helping with Adjustment

* MDS 3.0

* Care Planning

* Assisting with Medicaid Applications and other Financial Matters

* Advocacy

* Crisis Intervention

* Discharge Planning

To qualify for this Social Worker position, the Ideal Candidate:

* Must have a Social Worker Certification/License in good standing in the state of NJ

* Must be able to multitask and organize

* Must have a passion for the senior population

* Must have strong writing skills and be an effective communicator

* Must be a team player

Familiarity with Point Click Care (PCC) is a plus but not a requirement.
