QUALIFICATIONS:

§Licensed Practical nurse licensed in the state.

Hospice Experience preferred, but will train right candidate

§Excellent telephone skills and professionalism

RESPONSIBILITES (including but not limited to):

§Evaluate patient services provided by the interdisciplinary team.

§Serve as liaison to Administrator regarding clinical challenges and progress in patient care.

§Supervise the quality of care provided.Participate in orientation and education of all staff.Support all caregivers on the hospice interdisciplinary team including any contracted staff.

§Be available for on-call if needed.Provide support to on-call staff.

§Schedule and supervise meetings of the interdisciplinary team.

§Preparing all POC and Notes necessary or relating to IDG weekly meetings.

§Processing all orders and missed visits.

§Billing report

§Processing newly received signed orders and follow ups for non-signed orders.

§Scheduling RN and LPN visits

§Participate in orientation of new staff.

§Assist other members of the management team with delegated clerical functions.

§Open communication with Manager and all other team members.

§Participate in driving all quality assurance and performance improvement activities.

§Contribute to effective patient and customer relationships by assisting patients, families, physicians, and team leader to resolve expressed concerns.

§Facilitate interdepartmental communications with co-workers, direct report(s) and staff.

§Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.

§Responsible for assisting the program meet and/or exceed goals including census goals.

§Will be committed to ensuring that this hospice program provides care and services of the highest quality and safety and will continually strive to improve performance.

§Will be willing to serve as a member of the QAPI committee when asked.

§Will participate in the implementation of Quality Assessment and Performance Improvement projects as assigned by the Quality Assurance, Performance Improvement committee.
