Physicians Choice Home Health is recruiting for a Speech–Language Pathologist to join our Home Healthcare Team. In this role, you will be responsible for providing quality home health visits to our patients in the comfort of their home. You will be joining a team of compassionate and dedicated healthcare professionals who are committed to providing the highest level of care to our members.

The Speech–Language Pathologist is responsible for implementation of standards of care for speech language pathology services and for adherence to all conditions in the Service Agreement.

Objectives:
  • Conducts appropriate evaluations, assesses home environment (as appropriate), identifies equipment needs relative to speech/language function.
  • Identifies functional speech defects and establishes a plan of care to improve the patient's function.
  • Administers speech therapy program utilizing specialized therapeutic technique and/or equipment.
  • Recommends communication devices/aids as indicated.
  • Confers with referring physicians and other Physicians Choice Home Health personnel to ensure coordinated and comprehensive care contributes to the total plan of care.
  • As appropriate, participates in case conferences, in services and other meetings required to ensure coordinated and comprehensive care.
  • Prepares and submits notes per Physicians Choice Home Health policy.
  • Maintains clinical competency in speech language pathology practice and theory.
  • Identifies patient and family/caregiver needs for other services and refers as appropriate.
  • Instructs patient, family/caregiver and other Physicians Choice Home Health health care personnel in the patient’s treatment regime as indicated.
  • May supervise home health aide in procedures directly related to the speech–language therapy plan of care.
  • Performs other duties as required to facilitate the delivery of speech language pathology services.
  • Possesses a master’s or doctoral degree in speech-language pathology (SLP), and who meets either of the following requirements:
  • Is licensed as a speech language pathologist by the State in which the individual furnishes such service; or
  • If the State does not license speech-language pathologists:
  • Must successfully completed 350 clock hours of supervised clinical practicum (or is in the process of accumulating such supervised clinical experience);
  • Performed not less than 9 months of supervised full-time SLP services after obtaining a master’s or doctoral degree in SLP or a related field; and
  • Successfully completed an approved national examination in SLP
  • A minimum of one (1) year's clinical experience in Speech–Language Pathology.
  • Demonstrates good verbal and written communication, and organizational skills.
  • Possesses and maintains current CPR certification.
  • Ability to be able to travel to different patients' homes.
  • Discretion in dealing with sensitive and confidential information.
  • Prior experience working in a home health setting is preferred, but not required.

Rates

Physician Choice Home Health currently pays SLPs $60-95/visit and $40/hour for in-services, trainings, and orientations. Per visit mileage reimbursement is offered for W2 employees.Our Mission

Salary Range Disclaimer

The rates listed represent the low and high end of the range for this position. Visit and reimbursement rates vary based on visit type and employment status (1099 or W2). Rates may be adjusted in the future, and employees will be notified of these changes.
