Description

 The Healthcare Director is responsible for the overall management of care and services provided by the Wellness Department. This person will strategically lead their team to advocate for our resident’s health and wellness, manage, promote and coordinates quality care and services for our residents, and leads the delivery of quality Wellness services in accordance with company, federal, state and licensing regulations. The Healthcare Director will report directly to the Executive Director. 

• Overall management of care and services provided by the Wellness Department. Supervision and management of the Wellness Department’s team members. This will include replacing team members in the event of call offs; Management of department’s staffing hours.
• Document resident assessments.
• Document and complete Individualized Service Plans; Participate in resident care conferences.
• Determine appropriate care needs through assessment processes and management of Wellness services; coordinate healthcare delivery for each resident.
• Oversee, manage and coordinate the delivery of quality care/clinical care needs according to the resident’s Physician’s orders and plan of care, as well as, company, federal, state and licensing regulations.
• Communicate with physicians, pharmacists and other clinical providers to coordinate care and services for residents; Communicates care needs with family members.
• Champion the Medication Management program; Supervise and manage Medication Managers and LVN/LPN Medication Managers.
• Regulate the medication and health-related supply distribution process, including appropriate documentation, ordering, storage, security, destruction and pharmacy consultations.
• Coordinates services with contracted vendors to ensure department’s needs are managed.
• Supply ordering; Budget management.
• Manage daily assignments assigned in compliance with federal and state nursing requirements.
• Manage prevention of disease transmission and infection control for community.
• Work with the Director of Sales and other managers to assess prospective residents for move in.
• Maintain department compliance; Compliant with Federal, State/Provincial, board of nursing and other applicable regulations.

Requirements

• Current Good standing Licensed Vocational Nursing or Registered Nurse.
• Experience working in home health, assisted living, or long-term care.
• At least one year of experience in nursing management, daily operations, hiring and scheduling nursing staff.
• Proven knowledge of nursing practice as per federal and state board of nursing requirements.
• Proven experience in demonstrating the ability to analyze recorded data from clinical evaluations.
• Proficient in using the computer and ability to learn new applicable software applications.
• Availability to work weekends, evenings and as needed. This includes being on call.
