What You’ll Do The Wraparound Facilitator is guided by the wraparound philosophy that emphasizes empowering families. This is defined through the following value base:

Care for youth in the context of their families

Primary duties include:
Performs assessment of youth/family needs through the development of multi-perspective family story/timeline
Guides the family through the wraparound process.
Implements the four phases of wraparound; Engagement & team preparation; Initial plan development; plan implementation and transition.
Builds and develops child and family teams, facilitating monthly child and family team meetings and additional crisis meetings, as necessary.
Maintain necessary documentation regarding meetings and progress of the care plan.
Completes care plan, crisis plan, intervention plan, plan service authorization, minimum data set, eligibility reviews, and participant status with the designated system, TOBI.
Uploads all Monthly Child and Family Treatment Team documentation within TOBI system for DMHA review.
Uploads all Monthly Child and Family Treatment Team documentation within EPIC system for SEMHC coders review.
Review and incorporate DMHA Supervisor feedback to the team in an effort to increase the wraparound fidelity to the model.
Advocates for youth and their families through community agencies while adhering to the Wraparound principles.
Complete regular assessments utilizing the CANS Assessment tool and corresponding documents.
Develops comprehensive transition plans with discussion occurring in the team development phase to ensure adequate natural supports/family voice for long term success.
Maintains updated treatment plans with progress notes in the timeliness per agency guidelines.
Completes monthly client check within Indiana Medicaid Portal to verify program eligibility and status.
Minimum Hiring Requirements:
Bachelor's degree in Social Work, Psychology, Criminal Justice, or other related degree required
Minimum of two years of experience in working with children and families
Must possess or obtain wraparound DMHA Certification (will obtain during employment)
Must be eligible to become a certified CANS super User
Must have a valid driver’s license and proof of insurance
