What You’ll Do in the PATH Outreach Specialist position: The PATH Outreach Specialist helps homeless adults in our community live healthier and more fulfilling lives. The Outreach Specialist manages the care provided to clients, helps them gain access to needed services and teaches skills necessary for clients to achieve their goals. Primary responsibilities for the Outreach Specialist include:
Connecting with homeless individuals where they are
Determining the need for services (medical, social, education, etc.) using the Adult Needs and Strengths Assessment (ANSA)
Developing a specific treatment plan that supports the client’s goals
Helping the client obtain needed services, such as food or housing assistance, Medicaid, Medicare, Social Security benefits, transportation, skills training, workforce training, etc.
Providing one-on-one skills training, focusing on skills such as medication and symptom management, health and household management, coping strategies, interpersonal relationships and more
Advocating for the client and serving as a link between the court, healthcare providers, employers, community agencies and other Oaklawn services
Other duties as assigned
The PATH Outreach Specialist interacts directly with clients, outpatient therapists, physicians, other employees involved in the case, community agencies, referrals sources and other services. The frequency varies with need, availability of the client and the level of intensity, but usually ranges from several times per week to weekly and may include availability in the evenings and weekends. Minimum Hiring Requirements:
Associate degree in social / human services related field required. Starting pay rate is $15.93.
BA/BS in social service / human services related field preferred. Starting pay rate if $18.00.
Two years’ experience in a social service setting preferred (mental health preferred; experience with seriously mentally ill adults highly preferred)
Must have a valid driver’s license and proof of insurance
